MAST Individual Fundraising: Urban Ventures Coffee Sales

Instructions: Please complete a new form for each CUSTOMER.   Be careful to enter your data accurately.  Double check your work.  If the customer did not place an order for a particular item, you still need to make an entry.  If entering for Quantity, enter 0.   If the customer paid by check, please be sure to enter the check number at the end of the form.  You will be required to manually add up your orders.  Given that you are really smart, this should be no problem.  Be sure to check your totals.
