REGISTRATION
Registration should run smoothly if skiers follow this two step process:
Step One:
Complete and Return the MAST Registration Form, the MAST Assumption of Risk and Release of Liability Form, and the MAST Power to Authorize Medical Treatment Form (below).
Please carefully read, complete, and sign/date all three forms. Return all forms and the team fee of $450* made payable to Mpls Alpine to:

*If you are engaged in MAST fundraising, you do not need to pay the team fee in full by Oct. 22. Instead, please make a $75 NON-REFUNDABLE deposit to MAST by Oct. 22 to reserve your place on the team. You can pay the balance due once fundraising closes. The Season Pass fee is due in full by November 5.

Complete and Return the Minneapolis Public Schools(MPS) and Minnesota State High School League(MSHSL) Forms TO THE ATHLETIC DIRECTOR OF THE HIGH SCHOOL THAT YOU ATTEND OR TO THE ATHLETIC DIRECTOR OF THE HIGH SCHOOL IN WHOSE ATTENDANCE AREA YOU RESIDE. You can pick up paper copies of these forms from the A.D.’s offices at South, Southwest, and Washburn or click on the links below to print them yourself. PLEASE NOTE: If you played a fall sport for your high school, the only form you need to RE-SUBMIT is the Emergency Card. The other forms are valid for participation on the ski team.

For Washburn/South Students: You will need...

South Students can use the form available with this link. Washburn students need to pick up an edited version of this form from the office of the Washburn Athletic Director.

Pages 1-4 review eligibility rules as well as safety and concussion management policies and procedures.
Pages 5-7 require signatures, dates, and input of important biographical and health information. Note that parents also need to sign some of these forms. PAY ATTENTION to these forms. For some reason, it is easy to leave required spaces blank, which in turn could delay the start of your participation. Pages 5-7 need to be returned to your athletic director.
4. Physical Qualifying Examination Clearance
If you are new to high school athletics with MPS, you will need to complete this form and secure your physician's signature. If you played a fall sport for your high school (park board or middle school sports do not qualify), you will not need to re-submit this form. If you skied with MAST in past seasons and did not play a fall sport, you are STRONGLY encouraged to check with your A.D. to see if your Physical Qualifying Exam form is still valid. Last year, a number of MAST athletes were surprised to learn that their form had expired (it is only valid for three years) and were forced to sit out the start of the season. Don’t let that happen this year. The School/District/League forms are due November 1 for all returning MAST Athletes; November 5 for new MAST Athletes. However, you will make your athletic director's life a lot easier if you can submit them as soon as possible.

2012-2013 Fee Information

We invite you to join MAST
for the 2012-13 Season!
FEES
The two main fees all athletes pay are the TEAM FEE of $450 and the SEASON PASS FEE of $349. All fees are paid to Mpls Alpine. No fees are due to MPS for participating in MAST.
First, an explanation about the increased team fee: In response to feedback we received in the end-of-season parent surveys, we increased the team fee this year and eliminated the mandate for each athlete to fundraise a minimum amount. The new team fee comes closer to reflecting the "true cost" of team membership and is comparable to or lower than the fees charged by most club sports and dramatically lower than the fees charged by private ski racing teams. Fundraising opportunities are still in full force for those athletes and families who would like to fundraise and use the money raised to pay down their fees. Individual fundraising efforts will no longer fund the team's general fund, only TEAM fundraising efforts will be applied to the team generally, i.e., the State Fair Fundraiser. Instead, proceeds from individual fundraising efforts will directly benefit the athlete who participates. What you earn directly reduces your out of pocket costs. Fundraising has begun. So, here is a breakdown of the fees for 2012-2013.
$450: Team Fee to Mpls Alpine.
This fee covers most of our ongoing expenses for the season: coaching, equipment, administrative, and race entry fees and lift tickets.
$349: Unlimited Season Pass to Hyland Ski and Snowboard Area.
To receive this discounted season pass price, athletes need to purchase the pass through Mpls Alpine. Pass fees can be combined with the team fee in one check to bring the total up front fee to: $799. Checks can be made payable to Mpls Alpine. For returning athletes, the team fee is due October 22. For new athletes, the team fee is due November 5. The deadline for payment of the Season Pass Fee is November 5.
Our team is scheduled to assemble at Hyland on Monday, November 12 at 7PM to take season pass pictures and distribute season passes.
