FEES and REGISTRATION PROCESS
MAST Registration Process
Registration should run smoothly if skiers follow this FOUR-STEP process:
Step One: Complete an Online MAST Registration Form
Step Two: Submit Payment For The MAST Team Fee and Hyland Season Pass.
Step Three: Print, Complete and Sign the MAST Assumption of Risk and Liability Release and the MAST Power to Authorize Medical Treatment Forms. Parents and athletes must complete and sign TWO additional MAST Forms.
Submit these forms on Oct. 21 at the Parent-Athlete Information Meeting at SW High School.
Step Four: Minneapolis Public Schools(MPS) and MN State HS League (MSHSL) Forms In addition to the forms specific to MAST, you will need to complete, sign, and submit a series of forms specific to the Minnesota State High School League and to Minneapolis Public Schools.
Mpls Public Schools and the MN State High School League each have its own set of forms. Some of these forms are rather confusing to complete. Proceed carefully. Adding to the confusion is the fact that MAST athletes need to obtain or download the VERSION of the MPS/MSHSL form particular to the high school they attend or in whose attendance area they live.
The surest way to secure the correct forms for each school is to pick up the forms directly from the high school athletic director. If you are in high school, this should be an easy process. If you would like to access the forms online, follow the links below.
The Health Questionnaire is an important document that often gets overlooked because it seems to duplicate the information requested in the Qualifying Physical Examination Clearance form. The difference is that the Health Questionnaire needs to completed and signed EVERY year, whereas the Physical Exam form needs to completed and signed by a physician every three years.
Forms need to be read and completed carefully. Proper signatures need to be affixed (of both parents and students) to many documents. If anything is missing, your athletic director will ask you to correct it and your participation with MAST could be delayed.
This is A LOT of information. Please take the time to download and secure all the forms you will need to submit in order to participate in ski racing this season. Remember, MAST forms need to be turned into Dave Krueger. MPS/MSHSL forms need to be turned into your athletic director. MAST is not allowed to accept the MPS/MSHSL forms.

- If you have not already done so, complete and submit the MAST Team Registration Form. To access it, click HERE.
Step Two: Submit Payment For The MAST Team Fee and Hyland Season Pass.
- The Team Fee remains unchanged from 2012-2013: $450. All RETURNING MAST athletes are asked to pay the team fee IN FULL by October 21, 2013. The Team Fee is due in full for NEW MAST athletes by November 5, 2013. Checks may be made out to MAST
- The cost of a Hyland Season Pass also remains unchanged: $349. All RETURNING MAST athletes are asked to pay for the Hyland Season Pass by October 21, 2013. For NEW MAST athletes, the $349 season pass fee is due November 5. Checks may be made out to MAST
- For those athletes participating in INDIVIDUAL fundraising efforts (Hoigaards Tent Sale, Dunn Brothers Coffee Cards, Wreath Sales), a Team Fee Deposit of $75 is due October 21 for returning athletes.
- The Athlete Fundraising/Fee Balance Sheet (Click Here). Please print and complete this form and return it with your season pass and team fee payments. This will make our PARENT VOLUNTEERS in the MAST Finance Department happy and their work a bit easier.
- Send payment and the Athlete Fundraising/Fee Balance Sheet to: Susan Steen, MAST Treasurer, 2800 Brookwood Terrace, Mpls, MN 55410
- Submit the payment and Athlete Fundraising/Fee Balance Sheet on Oct. 21 at the Parent-Athlete Information Meeting at SW High School.
Step Three: Print, Complete and Sign the MAST Assumption of Risk and Liability Release and the MAST Power to Authorize Medical Treatment Forms. Parents and athletes must complete and sign TWO additional MAST Forms.

- Please read through these documents carefully. Be thorough and accurate in completing these forms. Write legibly. Note that BOTH parents/guardians are required to sign the Assumption of Risk and Liability Release form.
- Mail these TWO MAST forms (Assumption of Risk and Liability Release and Power to Authorize Medical Treatment ) before Oct. 21 to: Dave Krueger, MAST Team Registrar, 5109 Oliver Ave. S. Mpls, 55419

- For South HS Athletes: you can access the forms you need by clicking on each of the forms listed below. Turn these forms into the Athletic Office at South High School.
- Emergency Card (1 page)
- Media Release Form (1 page)
- Physical Exam (Sports Qualifying Physical Examination Clearance Form-4 pages).
- Athletic Eligibility Brochure (includes a list of general rules governing conduct and eligibility that parents and students are asked to review together; information on the signs and symptoms of concussions and MSHSL policy on concussion management; a MSHSL Eligibility Statement that students and parents read and sign; a MSHSL Annual Sports Health Questionnaire that parents and students complete and sign—many pages.)
- For Washburn HS Athletes: Download and print the Physical Exam and Athletic Eligibility Brochure linked above. Washburn made changes to the MPS Emergency Card and merged it with the Media Release form. This redacted form is only available from the Washburn Athletic Director’s Office. Here is a phone number for the Washburn A.D. office: 612-668-3457. Turn all forms into the Athletic Office at Washburn HS.
