Please click HERE to download and print the forms and to read detailed instructions for completing the forms.
Parents: be sure to read through these carefully and make sure you and your athlete have initialed and signed every form thoroughly. If you have questions, please talk with your athletic director or give Mark Conway a call at 612-275-8392. Submit all forms to your high school athletic director.

Season Pass Photo Night
The High School Alpine Season officially opens Monday, November 13. No practice will be held that day. Instead, we will meet at Hyland at 7PM to take your photo and pick up your Hyland Season Pass. Set aside one hour for this project. You will need to have a parent sign the attached Liability Release form and bring it with you to Hyland on the 13th.
Attached is a list of athletes who either ordered a Hyland Season Pass through MAST or will be securing a pass directly from Hyland. Every MAST athlete needs a season pass. If you still need to purchase a pass, please go to the Three Rivers Park District website to order your pass.

Dryland training begins Tuesday, November 14th. We will train at the Washburn Stadium, even in light rain or snow. We will move indoors in icy or stormy conditions... So, dress in layers. Wear athletic shoes and bring a hat and gloves and water.
If you still need equipment, call or email me.
I look forward to seeing all of our parents on Sunday.
